Please contact your child's Physician or your School Nurse if you have any questions regarding your child's Immunization Status.
PLEASE NOTE IMPORTANT REQUIREMENTS (AND RECENT CHANGES) for 7th GRADE AND 12 GRADE ENTRY!
7th Grade: IN ADDITION TO 7TH GRADERS NEEDING ONE DOSE OF Tdap, effective school year 2016-17, 7th graders will need one dose of Meningococcal Vaccine.
12th Grade: All students entering the 12th grade effective school year 2016-17 will need at least one dose of Meningococcal Vaccine administered on or after the 16th birthday.
