STUDENT INFORMATION CHANGES

To change and/or update your child's phone number, emergency contacts, etc., please make changes directly in Final Forms.

FOR ADDRESS CHANGES please go direcly to Final Forms and update your address. Take a picture of a a current proof of residency (gas/water/electric bill, lease agreement/mortgage statement, or bank statement) and upload to Final Forms or bring the proof of residency to your child's school.

The Early Learning Center is a “Five-Star” Step Up To Quality program (SUTQ). Our preschool program is designed to meet the unique needs of young children ages 3 - 5.

NEW STUDENT REGISTRATION K-12

All student registration is now completed through Final Forms. Please click on the link below for instructions, as well as the link for Final Forms.

REQUIRED DOCUMENTS

Please email or upload to Final Forms the following documents in order to complete registration:

Child's Original Birth Certificate

If you do not have an original, one can be obtained for $25 at the Ashtabula City Health Department, 4717 Main Ave. 440.992.7123. (Even if your child attended the Early Learning Center, you must bring your child's BIRTH CERTIFICATE with you!)

Immunization Records (Doctor's office can fax to: 440-992-1252)

Custody papers (if applicable)
- Must be date-stamped by the court!
- This includes all divorce, dissolution, separation, custody changes, guardianship, etc.

Proof of Residency
- Current utility bill, paycheck stub, rental/lease agreement (must have your name and address on it.)
- If you are living with or staying with another family, the head of household of that address must complete the Statement of Residency form and provide proof of residency. This must also be notarized by a certified Notary Public.
- If you move, you must show your address change confirmation from the US Postal Service.
- If you cannot prove residency in the Ashtabula Area City School District, your child may be denied enrollment. (See 'Homeless Students' in the AACS Bylaws and Policies)
